Government-funded CGM Application - BOOK NOW to lodge your application

On 1 April 2017 the Commonwealth government announced that children aged up to 10 are automatically eligible for government-supported CGM upon application through an endocrinologist, credentialed diabetes educator or nurse practitioner specialising in diabetes. If you are between the ages of 11 and 21, you may also be eligible for government-supported CGM, however additional eligibility criteria must be met.

Amy Rush CDE/APD is now taking clinic bookings for patients seeking to apply for government-supported CGM. Please click here for information regarding applications and the relevant form.
